Below you will find information regarding your new patient appointment.  

Please Bring 

Identification

We will need at least one form of ID on file for your first appointment. 

Dental Insurance Card

We know all insurance companies are not suppling ID cards now. Please have your member ID ready and a good phone number to contact your insurance provider. 

Your Smile 

Expect a few pictures! This can include intraoral photos, full mouth x-ray's, a panoramic x-ray. All this imaging is very important for accurate diagnostic documentation.

Medical History Information 

This includes a list of all current medications, a past medical history summary, current ongoing treatment, and names of physicians' who have preformed any past treatment. 

New Patient Paper Work 

You can email completed paper work to coxdental@pldi.net or bring to your appointment. 

Upon Arrival 

Please arrive 15 minutes early to all New Patient Appointments. And 5 minutes early for routine visits. Confirm appointments by phone or text 24 hours prior to appointment. Promptly notify about reschedules and cancelations.   

Insurance

Dr. Jim Cox is in network with the following insurance companies: Assurant, Aetna PPO, Blue Cross Blue Shield of Oklahoma, Cigna PPO, Delta Dental Premier Plus, Sunlife, United Health Care Medicare Complete, Health Choice, Mutual of Omaha. 

But please call to verify your policy.

By phone or in person. All major credit cards accepted including American Express.
